This is the first survey available in 21 languages in the history of Pennsylvania. Please find below the links for the survey in 21 of the most commonly read languages in the Asian Pacific American community.
The COVID-19 Vaccine Survey is conducted to find out how the vaccinations are going among Asian Pacific Americans (APAs) in Pennsylvania. Your participation will help stakeholders in healthcare and public service as well as government agencies to improve health equity by increasing understanding of vaccine progress and barriers in Asian Pacific American communities. The survey will also help us combat COVID-19 in the long run.
The survey consists of 13 questions. It will take about 10 minutes to complete. There will be FOUR sections:
- Basic Demographic Information
- Vaccine Progress
- Vaccine Barriers
- Vaccine Attitudes
The survey is anonymous and confidential, meaning no one will know who you are if you fill out the survey and we will NOT share any individual or personal data. We will only report the overall results of the survey, not individual surveys.
